So a small relief should be filed if a serration falls at the slot?

Yes, that is my understanding. Give Sam a call. He ran into this problem with the new KS that he and Mike Breeding were making. I think all of the above answers are quite good, but I thought one more might help you out.

I removed the bolt and cleaned it, the nut and washer and the slot in the lever. It grips the shaft much better, but I do need to replace the bush in the cover.
